September 1, 2024 - August 31, 2025

I (we), the parent(s) or legal guardian(s) give my (our) child(ren) permission to fully participate in any event sponsored by Living Hope Evangelical Free Church of Willmar (Living Hope). I understand that while my child(ren) participates in the events and activities, he/she is responsible to abide by the rules set forth by the leaders and supervisory personnel. Any serious infraction of rules and/or conduct by my child(ren) can result in dismissal from the event.

In case of emergency, I authorize an adult, in whose care my minor child has been entrusted, to consent to medical attention and/or x-ray examination. I understand that in the event medical intervention is needed, as soon as it is safe to do so, every attempt will be made to contact the person(s) listed on this form immediately. In the event that I cannot be reached, I hereby give permission to the medical professional selected by the leader to hospitalize and/or secure medical treatment deemed necessary. I understand Living Hope and its agents strive to implement reasonable safety precautions during all events and activities. I understand there is some inherent risk in participating in events and activities, as well as the possibility of unforeseen hazards. I agree not to hold Living Hope, its trustees, directors, employees, agents, representatives, and/or volunteer staff liable for any damages, losses, or injuries incurred by me or my child(ren).

I understand that video and/or photographs may be taken of activities sponsored by Living Hope. I give permission for all media (audio, video, and/or photographs) of my child to be used in church publications (bulletin, power point and/or bulletin boards, etc.) or on the Living Hope social media pages and website.  I will make no monetary or other claim against Living Hope for the use of such photographs or videos.
